Client Characteristics: Establishment of Blockchain based data interoperability platform for clinical data interchange – eVaccine & Health Information – Passport Management System

The Reference Client envisioned an eVaccine & Health Information – Passport Management System, designed to ensure stringent adherence to the framework for information management as expected by the HIPAA (Health Insurance Portability & Accountability Act) and GDPR (European Union’s General Data Protection Regulation) norms.

The solution leverages a Blockchain based approach delivering a compliant platform for data democratization that can be widely consumed by the ‘Information Consumers / Verifiers’ while being,

  1. Pseudonymous for ePHI protection
  2. Immutable and tamper-proof, maintaining data integrity as introduced ‘at source’
  3. Integrated with Heterogeneous Data Ecosystems with information exchange based on the healthcare standards driven by HL7 / FHIR for data interchange
  4. Portable based on predefined ‘levels of information-access’ controlled by the on-chain SmartContracts based on data privacy and protection laws
  5. Available, visible, and trusted with the distribution of anonymised data signatures creating an ecosystem of ‘Trust Owners’

The Reference Client aims to address white spaces that are more holistic and designed to be Patient Centric. These white spaces are expected to address the tactical needs (vaccination, travel, access to public spaces to name a few) in conjunction with strategic needs for detailed health information access – all working towards inclusive, affordable and result oriented healthcare services.

The solution shall also enable the need for supporting the Population Health services, thus, extending the care services to the care network across providers and across EHRs (Electronic Health Records) enabling care continuum for the Patient. Driven on the need for health data democratization, within the expectations of the regulatory frameworks:

  1. Changing regulations for health data interoperability – Impetus provided by the Trusted Exchange Framework and Common Agreement (TEFCA), a provision of the 21st Century Cures Act, US, is meant to advance the secure exchange of electronic health information and other data among health information networks
  2. Increasing focus on care continuum – for not only administrative needs (payors, verification) but for enabling the larger goals of care continuum has been gaining acceptance globally. The aim is to advance the use of trusted exchange of Electronic Health Records (EHR) and other health IT data
  3. Ensuring against security and privacy breaches – “Zero” laxity towards data integrity loss, data portability, data ownership & protection of the Personally Identifiable Information, and secure data interchange. Thus, the need for FHIR® (Fast Health Interoperability Resource) conformant API design mandated for all underlying Health Information Systems (HIS) and EHR for enabling interoperability
  4. Push to move from Payor / Provider Centric to Patient Centric – Health Information Exchanges (HIEs) implementations globally are in various stages of adoption with a focus on the needs of the Payor / Insurer – not the Patient. With impetus on Population Health, wider acceptance of telehealth and driven by the need for “meaningful use” for faster and accountable care, cross network access of health information (specifically for key information elements, e.g. as codified under US Core Data for Interoperability – USCDI) is gaining wider adoption

Our Approach to enabling the Business Drivers

Architected an eHealth Wallet based on VCI’s SMART Health card Framework – W3C Verifiable Credentials and HL7 on FHIR addressing,

  1. Individual / Information Owner Access – receives an eHealth Wallet based Passport Management System access on Mobile
    • PDF Share or QR Code based access for VIEW – Just the status view (Red:Amber:Green)
    • Ability to SHARE detailed information – Leverages an “Authentication Step” for the “Issuer” based on the rules codified on the “SmartContract” where the Individual / Information Owner grants permission to the “Verifier” to access the “defined health information based on Core Data” and leverages the FHIR based APIs (RESTful APIs) to SHARE from the Source System of the “Issuer”
  2. Health Passport System – is a Blockchain based Trust Ecosystem that maintains “SmartContract” driven trust framework aligned to the “Verifier’s” Persona (View, Level of View OR Share)
    • Privacy and Levels of Access – designed on the “Data Minimization” approach, aims at disclosing only “required information” preconfigured in the “SmartContract”
    • Data Interchange for Interoperability – based on the authorization by the Individual / Information Owner, the HL7/FHIR based APIs (based on matching the Blockchain ID and the Verifier system’s Patient ID) provide for a protocol driven access to information. The information elements use the standard industry ontology to ensure health systems / payor systems visibility

Outcome delivered

Development of the Enterprise Process Model and the aligned Enterprise Data Model as a precursor to establishing the sandbox (which is the work in progress) that,

  1. brings the Patient at the centre of health information interoperability – not just across payors, providers, and pharma
